Care2Prevent is committed to providing comprehensive care and prevention education for People Living with HIV (PLWH) and adolescents/young adults/adults who are most vulnerable to HIV infection. Our program also offers a variety of services to LGBTQ identified youth, as well as youth who are experiencing housing instability. Our holistic approach to prevention and treatment seeks to provide continuity of care, increase sexual health awareness and literacy, and reduce stigma among affected populations. We believe that we must provide comprehensive care for those living with HIV in order to prevent new cases of infection in those most at-risk. Our program functions within an integrative service model that emphasizes collaboration between medical providers/physicians, case managers, behavioral health staff, outreach/retention workers, and research staff.  

Care2Prevent provides counseling services to people living with HIV, as well as to individuals vulnerable to HIV, and LGBTQ-identified people on the south side of Chicago. Additionally, Care2Prevent offers a variety of community support groups, case management services, primary prevention care, HIV medical care, as well as outreach and prevention program including a weekly Youth Drop-in Center at the Washington Park Refectory and mobile HIV testing.
