Chicago House provides evidence-based, sex-positive, community- and client-centered HIV prevention, testing, and linkage-to-care services. Our services encourage practices to reduce HIV risk and sexual harm by affirming sexual desire and promoting care for the whole person. From providing risk reduction techniques to fostering relationships with medical providers, our staff are knowledgeable about the numerous ways to stay healthy and can help identify the plan that works best for you.
In addition to offering HIV Counseling, Testing and Referrals, Chicago House’s HIV Prevention Department also provides linkage and retention in care services. With an estimated 71-81% of HIV-infected persons in the US lost to care and not having achieved viral suppression, it has become more important than ever to re-engage these individuals with a medical provider.
Our Linkage to Care Program works with HIV-positive individuals who are newly diagnosed or who have fallen out of medical care. Trained Care Coordinators work closely with each individual to identify and overcome barriers to engaging in care. Care Coordinators also provide accompaniment to medical appointments and help participants link to case management and ancillary services. The Linkage to Care Program has established a referral network with over thirty Chicago-area providers, including hospitals, community-based organizations and community health centers.
