Howard Brown is partnering with Rush University Medical Center, CJE SeniorLife, Midwest Palliative & Hospice Care Center and Heartland Alliance to design and develop a comprehensive health program for lesbian, gay, bisexual, and transgender older adults. With support from The Baxter International Foundation and Chicago Community Trust, our program for older adults — AGING AS WE ARE: It’s Our Time –is creating a new model of care for LGBT older adults in Chicago.

AGING AS WE ARE offers:

– A Geriatric Medicine Physician Specialist
– An On-Site Walgreens Pharmacy & Prescription Drug Assistance
– Mental Health Services
– HIV/STD Counseling and Testing
– Caregiver Support for your Partner or Primary Caregivers
– Legal Assistance including Advance Directives

When you call us, you will find a team that offers you respect and confidentiality. Come and experience a new model of care for LGBT older adults in the city of Chicago.

We accept most insurance plans, including Medicare/Medicaid.
