The Ryan White Part D Program is a long running, program of Howard Brown Health Center, which serves HIV+ adult women, transgender women, children, infants and adolescents (ages 13 -24). Participants work closely with a multi-disciplinary team of medical and mental health providers, as well as HIV medical case managers.  Case managers in this special populations program have focused areas of expertise, and provide strengths-based case management to all participants.  The program currently employs 2 case managers working specifically with HIV+ adolescents, 1 case manager working with HIV+ cis-gender women, and 1 case manager working with HIV+ transgender women.
