At The Care Plan, we cultivate a culture of client-directed care. Our client-directed approach to care means that we keep you firmly rooted at the center of health and aging decisions. We listen first, share expertise and advice on your aging future. Prepared with knowledge and perspective gleaned from our years of expertise, you are able to anticipate potential needs and plan accordingly. It is impossible to have your wishes executed if you have not prepared adequately to age. Failure to plan ahead generally results in choosing the path of least resistance when faced with a health challenge, and it is easy to fall between the cracks.

Aging Services include: Identity, health profile, care needs, support system, end-of-life wishes, legacy planning, financial resources, advanced directives, and legal protection.

All of our services are tailored to meet your individual needs, however, these are the areas we find our LGBTQI-identified clients most often desire assistance and support: Same-Sex relationship rights, homophobia & trans phobia discrimination prevention, HIV discrimination prevention, trans & gender spectrum 101, pronoun preference & name enforcement, and family of choice.

We work to provide affirming care to transgender and gender expansive individuals who seek our services. We recognize these individuals often have experienced barriers to quality health and aging services due to discrimination, ignorance, and intolerance. We explicitly educate our practitioners on harm reduction.
